  • This registration category is ONLY to be used by Active members who submitted a $250.00 deposit for Pre-Registration. Otherwise please use the Active Member Registration Category.

    The $250.00 deposit has been credited to the Conference Registration fee in this category.
    All other Active Members should register in the separate Active Member category.

    The Registration fee includes the Friday and Saturday scientific sessions as well as the Friday evening Welcome Reception. Guests of Pediatric Dentists registered in this category must register and pay for social events at the time of registration at the fees indicated below. Please note: if you are registering staff, you may do so on this form. You do not need to use the separate Staff Registration Form. The Registration Fee for Pre-Registered Active CAPD Members is $545.00 ($795.00 less the $250.00 pre-registration deposit that you already submitted) until June 30, 2023 and then $710.00 from July 1. ($960 less $250.00)///

    Cette catégorie d’inscription s’adresse UNIQUEMENT aux membres actifs ayant versé 250 $ pour l’inscription prioritaire. Le montant de 250 $ a été déduit des frais d’inscription au congrès dans cette catégorie. Tous les autres membres actifs doivent s’inscrire dans une catégorie distincte de membres actifs. Les frais d’inscription incluent les séances scientifiques du vendredi et du samedi, ainsi que la réception de bienvenue du vendredi soir et le dîner du président du samedi soir. Les dentistes pédiatriques inscrits dans cette catégorie doivent inscrire leurs invités aux activités sociales et payer les frais indiqués ci-dessous au moment de leur propre inscription. Prière de noter que si vous inscrivez des membres de votre personnel, vous pouvez le faire sur le présent formulaire. Il n’est pas nécessaire d’utiliser un formulaire d’inscription distinct pour votre personnel. Les frais d’inscription des membres actifs de l’ACDP ayant bénéficié de l’inscription prioritaire s’élèvent à 545 $ (déduction faite du crédit de 250 $ de l’inscription prioritaire) jusqu’au 30 juin 2023, puis augmenteront à 710 $ à compter du 1 juillet.
